Serving Butler and Warren County Children will be taking applications on the following dates:
Oct. 22, 2007 - 2:00 - 6:00 P.MOct. 23, 25, 26 and 27th- 9 :00 A.M.- 6:00 P.M. At 65 S. Main St. 2nd floor of the Commercial Building, Middletown, Ohio 45044.
Nov. 26, 2007 - 2:00- 7:00 P.M.
Nov. 27- 10:00 A.M.- 7:00 P.M
Nov. 29- 10:00 A.M.- 7:00 P.M.
Nov. 30 - 1:00- 6:00 P.M.
At the Franklin Public Library

You must reside in Butler or Warren County to receive assistance, please bring the following to apply:
Picture I.D for applicant
Proof of Address
Children's Birth Certificates
Proof of Income
Medicaid, Caresource or Amerigroup if received
If Foster Parent or Guardian, court documents are needed.
We assist with children ages newborn to 13 years of age.
You cannot be receiving toys from another agency or resource.
CHILDREN MUST RESIDE IN THE APPLICANTS HOME FULL-TIME
YOU MUST HAVE ALL DOCUMENTS AT REGISTRATION,OR YOUR APPLICATION WILL NOT BE ACCEPTED.
