Registrations for the Butler/Warren County will take place for the 2008 Christmas season:
Friday November 28- (1:00 P.M till 7:00 P.M)
Saturday November 29- (9:00 A.M till 5:00 P.M)
Sunday November 30- (12:00 P.M till 5:00 P.M)
Wednesday December 3- (9:00 A.M till 5:00 P.M)
Thursday December 4-(4:00 P.M till 8:00 P.M)
Friday December 5- (12:00 P.M till 7:00 P.M)
Sunday December 7-(12:00 P.M till 5:00 P.M)
Please come on these dates and times, as we are not open other days. These registration dates are open to both Butler and Warren County residents.
You MUST bring the following documents to apply:
Applications will not be accepted without required documents.
Photo I.D for applicant
Address verification
Proof of Income
Birth Certificates for all children
Medicaid, Caresource, Melina, or Amerigroup Cards if received.
Children must live in your home full time; you must have custody of children. If child has been placed in your home by the court system please bring verifying documents.
